| Registration Procedure for 2009 PCIC Technical Conference
PLEASE REGISTER EARLY!
The process to be followed is outlined below:
- All attendees must reserve hotel rooms directly with the Conference hotels. We recommend that hotel reservations be made PRIOR TO registering for the conference as registration fees are higher for those not staying at the Conference hotels.
- Please use the following Group Code for hotel reservations: IEE
Once hotel reservations are made, be sure to record the hotel confirmation number.
- To receive the special conference rate of $196 (USD) per night for a single or double occupancy room book prior to August 14, 2009.

- When registering for the conference a valid hotel confirmation number is required or an additional $200 fee will apply. No after-the-fact rebates will be issued.
- For conference registration by mail, courier, or fax, use the registration form in this program booklet or go on-line as described in #2 above and print a copy of the registration form. Be sure to make hotel reservations first using the above Group Code. Record the hotel confirmation number, and then register for the conference using the printed form, which contains directions for mail, courier, or fax submission. When registering for the conference, the hotel confirmation number is required or an additional $200 fee will apply. No after-the-fact rebates will be issued.

Payment:
- The following credit cards may be used for payment: MasterCard, Visa, or American Express. Check or money order should be made out to: 2009 IEEE PCIC ANAHEIM CONFERENCE
- All registration fees for the conference are in US dollars.
- Conference registrations received no later than July 1, 2009 will be at the regular conference rate. Conference registrations received after this date, but no later than September 7, 2009 will be at the regular conference rates, plus an additional $75.00 USD fee. Conference registrations received after September 7, 2009 will be charged an additional $75.00 USD from regular conference rate. The hotel must receive hotel reservations no later than August 21, 2009 in order to receive the special conference rates per night, single or double occupancy. After this date, the special conference rate is not guaranteed.
- All requests for registration changes, additions, deletions, or cancellations must be made either by email from the on-line registration website or direct to MicroSpec by fax (1-888-780-0663 or 416-780-0290), or by phone (1-888-780-9825 or 416-780-9825). Changes, additions, and deletions will be made whenever possible. Provided the email or fax notice of registration cancellation is received on or before August 15, 2009, a partial refund of the registration fee will be issued by check after the conference concludes. Cancellation requests received after September 1, 2009 will not be honored; however, a copy of the Conference Record on CD will be mailed to the registrant.
